Creatine: Is It Safe? Benefits, Dosage and the Myths, Explained
Walk into any gym in an Indian metro right now and you'll hear the same word in every third conversation: creatine. It's gone from a niche bodybuilder supplement to the default recommendation for anyone who lifts — helped along by Instagram reels, gym trainers, and a supplement market that's growing faster in India than almost anywhere else.
The catch is that the hype has outrun the information. Ask around and you'll hear it "damages your kidneys," "is basically a steroid," "causes hair loss," or that you must "load" it to see results. Almost none of that holds up. Here's what creatine actually is, what the evidence supports, and how to use it sensibly.
What creatine actually is
Creatine is a compound your body already makes (mostly in the liver and kidneys) from amino acids, and stores in your muscles as a quick-access energy reserve. You also get small amounts from meat and fish. Supplementing simply tops up your muscles' stores beyond what a normal diet provides, so they can produce short bursts of energy — the kind needed for a heavy set of squats or a sprint — slightly more efficiently.
Creatine monohydrate is the form used in almost every published study. It has roughly 30 years of research behind it, which makes it one of the most-studied supplements that exists — far more evidence than most things sold next to it on the shelf.
Why it's suddenly everywhere in India
This isn't a manufactured trend. Gym membership in urban India has been climbing at a steady double-digit pace for a few years now, and creatine has become the entry-level supplement recommendation for new gym-goers alongside whey protein — cheap per serving, backed by real research, and easy to explain in a 30-second reel. Sports-nutrition brands built for the Indian market have driven a large share of that growth, which is also why quality and pricing vary a lot between brands.
Popularity isn't the same as understanding, though — which is where most of the confusion below comes from.
What the evidence actually supports
- Strength and power. This is creatine's best-established benefit. Across hundreds of trials, people doing resistance training who supplement with creatine gain more strength and lean mass than those who train without it. The effect is real but incremental — it makes your training more productive, it doesn't replace training.
- High-intensity performance. Short, repeated bursts of effort — sets in the gym, sprints, jumps — are where creatine helps most. It does little for pure endurance activities like long-distance running.
- Recovery between sets and sessions. Some evidence points to slightly reduced muscle damage and faster recovery, letting you train a bit harder, a bit more often.
- Emerging areas. Newer, smaller studies are looking at creatine's effect on brain energy metabolism, mood and sleep-deprivation resilience. This research is genuinely interesting but early — not a reason to buy it if building muscle isn't your goal.
It is not a testosterone booster, not a fat burner, and not a substitute for protein intake or progressive training. Anyone marketing it that way is overselling a well-evidenced supplement into something it isn't.
The kidney myth, debunked
This is the single biggest fear stopping people from trying creatine, and it's built on a mix-up. Creatine is the supplement. Creatinine is a waste product your muscles produce as a byproduct, which your kidneys filter out — and it's the number doctors check on a routine kidney-function blood test.
Taking creatine modestly raises creatinine levels, because you're feeding the muscle system that produces it. A doctor unfamiliar with this can see a slightly elevated creatinine reading and flag it as a kidney concern, when it's actually an expected, harmless side effect of supplementation in someone with normal kidney function. Large reviews of the safety literature — including studies following people for several years at several times the typical dose — have found no evidence that creatine harms healthy kidneys.
The exception: if you already have diagnosed kidney disease, or take medication that affects kidney function (certain painkillers, some blood pressure drugs), talk to a doctor before adding creatine — not because it's proven harmful, but because your kidneys are already working with less reserve, and it's worth a two-minute conversation rather than a guess.
The hair loss question
You'll find this one repeated with total confidence in gym WhatsApp groups: "creatine causes baldness." The actual evidence is a single small 2009 study of 20 college rugby players, which found an increase in a hormone called DHT (the same hormone involved in male-pattern baldness) after a creatine loading phase. The study did not measure whether anyone's hair actually thinned, and no follow-up study has clearly reproduced the finding in the years since.

"Isn't it basically a steroid?"
No — and this confusion causes real hesitation. Anabolic steroids are synthetic versions of testosterone that change your hormone levels and carry serious medical risks. Creatine is a naturally occurring compound your own body already produces; supplementing simply increases the amount stored in muscle for energy use. Creatine monohydrate is also legal and FSSAI-regulated in India, sold openly over the counter, and it does not appear on anti-doping banned-substances lists — none of which is true of actual steroids.
How to actually take it
- Pick monohydrate. It's the most-researched form and the cheapest per effective dose. Flashier forms sold as "faster absorbing" or "no bloating" (HCl, buffered, ethyl ester) have little evidence of being any better — you're mostly paying for marketing.
- Skip loading if you want to. A loading phase (around 20 g a day, split into 4 doses, for 5-7 days) fills your muscle stores faster. A steady 3-5 g a day gets you to full saturation in about three to four weeks with no loading needed. Neither approach is "wrong" — loading just gets there sooner.
- Maintenance dose: 3-5 g a day, taken any time, with or without food. Consistency matters more than timing.
- Stay hydrated. Creatine pulls water into muscle cells, which is part of how it works — drink normally throughout the day, especially if you train in Indian summer heat.
- Buy tested brands. Look for micronised creatine monohydrate with visible batch testing or a purity certificate. Established marketplaces make it easier to compare labelled brands side by side than buying loose powder from an unverified seller.
Side effects and who should be careful
Creatine is generally well tolerated. The most common complaints are mild stomach discomfort or cramping during a loading phase (splitting the dose across the day usually fixes this) and a small amount of water-weight gain in the first couple of weeks, which is expected and not fat gain.
Check with a doctor first if you:
- Have any diagnosed kidney or liver condition
- Take medication regularly, especially for blood pressure or kidney-related conditions
- Are under 18 — most research is in adults, and a doctor is the right person to advise for teenagers
- Have any chronic medical condition you haven't discussed supplements with your doctor about yet
None of this makes creatine risky for the average healthy adult — it just means the handful of people who need a doctor's input should actually get it, rather than guessing.
The bottom line
Creatine monohydrate is one of the few gym supplements where the hype and the evidence roughly line up: it modestly but reliably improves strength and training output when paired with actual resistance training. The kidney-damage and steroid comparisons don't hold up under scrutiny, and the hair-loss link rests on one small, unreplicated study. If you train regularly, 3-5 g a day of tested monohydrate is a reasonable, low-risk addition — bought from a labelled brand, taken consistently, with the same training and sleep habits doing the actual heavy lifting.
